Inmate Smuggles Contraband from Law Library

At approximately 1430, I went to [Housing Module A] to look at the telephone access for segregation inmates. While standing in front of the law library room, [Inmate 1] stated the cable for the printer was missing. I entered the Law Library room and inspected the printer and computer to verify the cable was gone.

Inmate Surprises and Assaults Corrections Deputies

On 3/15/17 at around 0100, [an inmate] was out on his recreational time in [housing unit A]. When [Deputy 1] was just completing a module check, [the inmate] ambushed him by suddenly punching him in the face, knocking [Deputy 1] down. The inmate continued the vicious assault, then disengaged.

Correctional Officer Goes Down Transporting Maximum Security Inmate

Two corrections officers were assigned to transport a maximum security inmate to the local hospital for a doctors consultation in the ER. The inmate was secured to the gurney with handcuffs and leg irons. Corrections officer (CO) 1 was standing in the doorway while CO 2 was just inside the doorway of the the room near a counter and sink. CO 2 started to lean towards the wall and collapsed onto the floor, striking his head on the counter as he fell.

Inmate Attempts to Disarm Officers at the Hospital

In February 2013, at approximately 1608 hours, I received a call from Officer #1 reporting that an inmate was just taken down to the floor by him and Officer #2 after the inmate tried to take his weapon.

Inmate Attempts to Take Officer’s Gun in Hospital Bathroom

In July 2013, at approximately 7:15 am, an inmate was escorted from [Correctional Institution] to [Local Hospital] by two officers for a scheduled clinical appointment. At approximately 11:45 am, the inmate requested to use the restroom. Officer #1 searched the restroom and discovered a cell phone and tobacco product inside. Officer #2 confiscated the items and cleared the restroom for use by the inmate. The inmate again expressed his immediate need to use the restroom throughout the search. Officer #2 escorted the inmate inside the restroom. Once inside, Officer #2 removed the restraints from the inmate, freeing the inmate’s right hand.

Transfer of Unsearched Suspect between Agencies Leads to Suspect’s Suicide

A suspect was arrested after a foot chase and shooting a Deputy. He was arrested by a different agency and handed off to the investigating agency. During the transfer, officers on the scene failed to notify officers from the investigating agency that the suspect had not been properly searched.
