Tear Gas Sets Trailer on Fire During SWAT Operation

Police were dispatched for a noise complaint in a trailer park. When the responding officers arrived on scene, the subjects ran inside their trailer and locked the door. The officers tried unsuccessfully to convince the subjects to exit the trailer. The officers contacted their sergeant, who then contacted the duty lieutenant. The lieutenant contacted the chief of police, and the two decided to deploy the department’s SWAT team to handle the situation.

Failed Equipment Results in Officer Drowning

An experienced police diver was practicing routine underwater search-and-rescue maneuvers as part of a dive-team training exercise when a series of equipment failures resulted in the officer drowning.

Tasered Suspect Shoots and Kills Officer During Felony Warrant Service

Two officers observed a male felony warrant suspect walking along a downtown street in the nighttime hours. The suspect had a lengthy criminal history, which included prior assaults on police officers. As the officers attempted to effect the arrest, the suspect refused to comply with the officers’ commands to show his hands.

Accidental Use of Live Rounds

An officer was dispatched for a subject behaving erratically near a school, possibly armed with a knife. The responding officer was less-lethal trained (a 12 gauge shotgun, painted a different color, but capable of firing 12 gauge rounds). Upon arrival of the officer, the subject fled, and the officer tried to use his less-lethal shotgun on the subject.

Inmate Attempts to Disarm Officers at the Hospital

In February 2013, at approximately 1608 hours, I received a call from Officer #1 reporting that an inmate was just taken down to the floor by him and Officer #2 after the inmate tried to take his weapon.

Near Collision Between Police Helicopter and Drone

A state law enforcement officer was flying a helicopter over a major state highway at night in search for a stolen vehicle. While the pilot was searching, the pilot noticed a red light appear in front of the helicopter’s cockpit window.

Police K9 Bites SWAT Officer During Operation

A K-9 officer took part in a drug raid. Prior to the raid, the same K-9 and officer were involved in K-9 training. The training involved bite training and command/obedience training. The K-9 and officer went directly from training to the raid. As the K-9 officer exited his patrol car, he was unable to grab his K-9’s collar as he normally does.

Inmate Attempts to Take Officer’s Gun in Hospital Bathroom

In July 2013, at approximately 7:15 am, an inmate was escorted from [Correctional Institution] to [Local Hospital] by two officers for a scheduled clinical appointment. At approximately 11:45 am, the inmate requested to use the restroom. Officer #1 searched the restroom and discovered a cell phone and tobacco product inside. Officer #2 confiscated the items and cleared the restroom for use by the inmate. The inmate again expressed his immediate need to use the restroom throughout the search. Officer #2 escorted the inmate inside the restroom. Once inside, Officer #2 removed the restraints from the inmate, freeing the inmate’s right hand.

Less Lethal Shotguns Just in Time

Officers were dispatched to an unknown situation on a 9-1-1 call. Upon arrival they were confronted by a man with two butcher knives standing on the front porch of a residence. The suspect began to advance on the officers, two of whom had their handguns pointed at the suspect. The third officer deployed a less lethal shotgun, which the department had trained in and issued approximately 2 weeks prior to the incident.

Active Shooter Hoax at University

Communications received a text report of an active shooter occurring at (Major University) main group building. Incident Command made the decision to enter the building to search for the shooter. Responding officers from the local police department and the university police department formed into teams to begin the search. After a 2.5 hour search of multiple buildings with multiple teams, no shooter was found. The incident was determined to be a hoax.
